The primary purpose of this job is to perform the duties of MDS coordinator and to develop the MDS/CAA and Care Plan for all resident s on the skilled units of the Health Center.
Other duties included in this position are those of RN supervisor.
This position will supervise Moosehaven's skilled nursing facility reporting to DON.
The candidate for this position will be self-directed, energetic and professional managing resident-focused care.
Prior experience with MDS/Care Plan experience is REQUIRED.
Experience with MS OFFICE and Point Click Care software is required.
Minimum Requirements Must possess, at minimum, a Nursing Degree from and accredited school of nursing 2 years' RN experience MS Office and Point Click Care software experience is required Prior experience with MDS / Care Plans is preferred 2 years' experience as a supervisor in a hospital, long term care facility, or other related health care facility Must be able to carry the company phone and be on call on a rotating basis Job Type:
